Whether
you have a scheduled appointment or you are a “walk-in”
at the Piedmont Imaging Center you will be asked to bring
the following:
•
X-ray Orders (if your physician did not call it in by phone)
• Insurance cards: You will need to bring your insurance
cards even if a copy had been made on a previous visit.
You
will also be asked to fill out a registration form, sign
an insurance waiver form and any additional forms as needed.
You
will be asked to pay your insurance co-payment after the
procedure. If you do not have insurance, you will be told
the cost of the procedure. Payment is expected at the time
services are rendered unless arrangements have been made
in advance.
If you
would like to make a payment in person at our imaging office
please see the receptionist at the front desk. If you have
questions concerning your bill, you may call the number
printed on your statement to talk to a billing clerk.
If you need to pick up x-rays to take to your physician,
we encourage you to call 48 hours in advance so that we
may have them ready for you.
